Need a graphic designer for lead generating checklist

I want to create a checklist PDF file that can be interacted with.There will be a cover page and the second page will have the list.I have the brand guide for the general look and feel.Would like the second page to have a bar at the top with branding as well should users decide to forego the cover page when printing and downloading.
